The following information is being collected to assist National 4-H staff and others with securing additional financial resources to support 4-H Afterschool at the state and local levels. The information will also be used to help tell the 4-H Afterschool story to current and potential donors, parents, and the general public. Please take a few minutes to complete and return this information. The information can be completed by any Cooperative Extension faculty/staff (4-H, FCS, or others involved with 4-H Afterschool work) at the county and state levels or completed by a state-based faculty member that includes data for the entire state.
